St. Mary's Medical Clinics to offer sports physicals
The St Mary’s Hospital Medical Clinics will be offering Sports Physicals during the first full week in June.  This is the tenth year they will be offering the reduced rate for the physical exams.
The Nezperce Clinic will offer their sports physical program on Monday, June 6; Kamiah Clinic on Wednesday, June 8 and the Cottonwood Clinic on Thursday, June 8.  Students from any of the area schools are welcome to receive their physical at any one of the clinics.
 “We offer sports physicals at the reduced rate of $25 and return $15 to the schools for use in their athletic and academic programs,” said Jeanette Gorman, SMHC Community Relations Coordinator.  “Students need the completed forms on file before they can practice with their teams in the late summer.”
   Students entering the 7th, 9th and 11th grade and all transfer students must have a completed sports physical form on file with the school prior to their team’s first practice.
 Students should bring the Health Examination and Consent form completed on the front side and signed by a parent or guardian.  They should also bring their immunization records and a completed consent for immunization form signed by a parent or guardian.  Both forms are available from the clinics and the schools.  Immunizations will be provided at no charge during the scheduled Sports Physical clinics.  A charge will be assessed if students have their physical at another time.
