SMHC announces sports physical clinics for 2009
St. Mary’s Hospital and Clinics has scheduled their sports physical clinics for 2009.  They will be held from 5:00 to 6:00 p.m. on Tuesday, June 2nd in the Kamiah Medical Clinic; on Wednesday June 3rd in the Cottonwood Clinic and on Monday, June 8th at the Nezperce Clinic.    Lorraine Nuxoll, RN, is one of the many SMHC nurses involved with their annual sports physical clinics.  This year’s clinics have been scheduled at the beginning of June at their three satellite medical clinics.The $25 charge is payable at time of visit.  SMHC donates $15 back to the schools to help support their athletic programs. No appointments are necessary and any student can be seen at any clinic regardless of the school they attend or the community in which they live.
“If students attend the sports physical clinics we’re offering at the beginning of June it is a true cost savings for them, plus their schools benefit, also.  SMHC does this as a public service,” said Theresa Uptmor, SMHC Business Services Director.  “We can certainly perform the sports physical exams at any time in any of our clinics, but if a student has it done during one of our three established times we can offer the reduced rate and donate back to their school.”
According to the Idaho School Activities Association, students entering 7th, 9th or 11th grade and all new students must have a current sports physical form on file with the school.  SMHC will distribute sports physical packet to each of the schools in the area for students planning on sports involvement during the 2009-2010 school year.  Students in the 6th, 8th, and 10th grades should be bringing those packets home.  Each clinic also has extra copies.  The packet contains forms requiring parental signatures including a right to treatment form, a HIPAA statement allowing the clinic to forward the information to the school and a list of immunizations. Immunizations will be offered during the sports physical clinics at no cost.
Students should fill out the front side of the Idaho Health Examination and Consent Form and bring that form and the others to the clinic.  Students should wear shorts and loose fitting clothing and bring their glasses or contact lenses.
