Kindergarten screening for the 2015-2016 school year will be held at the Prairie Elementary School at 1916 East Street on Tuesday, April 7th and Wednesday, April 8th. This will take about 40 minutes for each child. Please bring your child’s state birth certificate. You can find the information and the form at http://www.healthandwelfare.idaho.gov/Health/VitalRecordsandHealthStatistics. Also bring your child’s current immunization record. State law requires each child to be fully immunized before the first day of school. Your child must be 5 years old before September 1, 2015 to enroll in school.
